ABSTRACT

As the population ages, the prevalence of chronic diseases increases and, accordingly, the probability of simultaneous occurrence of many chronic diseases among the elderly population increases, which affects the dynamics and prognoses of the underlying disease. Prevention of complications of concomitant semantics for the underlying disease seems to be a complex medical problem, the solution to which is aimed at improving the quality of life of patients. Based on the data obtained, the effect of polymorbidity on the course of acute lower limb ischemia (ALLI) was studied in the elderly urban population. The purpose of the study is to analyze the effect of multimorbidity on the life quality of the elderly urban population, exemplified by patients with acute lower limb ischemia. MATERIAL AND METHODS: 91 patients with ALLI (n=91) were examined, the mean age equaled to 70±1.2 years, with thromboembolism against the background of arterial occlusion. RESULTS: The age of patients is one of the highest risks and must be taken into account when deciding on surgical intervention and further therapy for ALLI. RESEARCH LIMITATIONS: The study had no administrative restrictions. CONCLUSION: It was found that the value of the comorbidity index in patients with acute lower limb ischemia is associated with the severity of the underlying disease and characterizes the predicted decrease in the probability of a 10-year survival and the risk of death.

ABSTRACT

A rare case of primary renal pelvis melanoma in 47-year-old man is presented in the article. Before surgery a patient was considered to have metastatic urothelial carcinoma. A diagnosis of malignant melanoma was based on immunophenotyping and detection of intracellular melanin pigment both in pelvis tumor and lung metastasis. The primary localization in the pelvis was proven by the presence of scattered melanocytes within urothelium.The patient had no previous history ofskin or mucosa melanoma. This is the sixth case of renal pelvis melanoma published in PubMed.

ABSTRACT

AIM: The study aimed to analyze the diagnostic performance of preoperative multiparametric magnetic resonance imaging (mp-MRI) in the staging of prostate cancer (PCa) versus postoperative histological examination and determine the most sensitive pulse sequence from the mp-MRI protocol in estimating the local extent of PCa. MATERIALS AND METHODS: The study comprised 112 men aged 52 to 84 years with a morphologically verified diagnosis of prostate cancer. All patients underwent pelvic mp-MRI before radical prostatectomy (RPE) no earlier than six weeks after the prostate biopsy. Radical prostatectomy was performed within two weeks after mp-MRI. MP-MRI findings and the results of postoperative histology were compared using a binary logistic regression model. RESULTS: The sensitivity, specificity, diagnostic accuracy, positive (PPV) and negative (NPV) predictive values for predicting extracapsular extension were 87.5, 92.6, 91, 84 and 94.3%, respectively; for predicting seminal vesicles invasion, they were 85, 95, 90, 80.9 and 96.7%, respectively. When stratified by the presence or absence of the pseudocapsule invasion, the reliability of detecting the tumor spread for different types of images decreases in the following order: DWI - T2 + DWI - T2 VI - DCE-MRI. CONCLUSION: mp-MRI has high sensitivity, specificity, general diagnostic accuracy, high NPV, and PPV values in detecting an extracapsular extension of prostate cancer. According to the binary logistic regression model, the greatest contribution to the decision on the presence or absence of extracapsular extension is also made by the DWI.

ABSTRACT

Reviews integrate the data from original sources of information on the given topic. The quality of the review of literature depends on the quality of initial information. The development of evidence-based medicine demands higher standards from clinical trials. The authors analyse available information on the treatment of erectile dysfunction basing on standard international requirements for clinical trials. A checking list is available for standardization of clinical assessment. The data obtained is ranged by the degree of statistical significance. Basing on the evidence-based approach, the conclusion on efficacy of the following modern drugs in the treatment of erectile dysfunction are made: sildenafil, iochimbin, prostaglandin E1 and tadalafil. The conclusion on efficacy of the other drugs against erectile dysfunction is impossible in view of methodological drawbacks of the trials.

ABSTRACT

The choice of the method to treat erectile dysfunction is often not easy. The evidence-based approach to the critical appraisal of the medical literature could help us to find the best way. All the available information was critically assessed in order to find the answer to the question: Ds sildenafil effective in the treatment of erectile dysfunction in patients with diabetes mellitus? All sources of information have been analysed including medical text books and the sources from the university of Cochrane library. The result of the analysis was the following: sildenafil is effective with a B degree of significance in the treatment of erectile dysfunction in patients with diabetes mellitus. This allows us to recommend sildenafil for treatment of erectile dysfunction in men with diabetes mellitus. The above algorithm of the critical assessment of the primary medical literature can be used for preparing standards of urological diseases treatment.
